Terms of Service Agreement for Connecting 2 Minds

Welcome to Connecting 2 Minds, a psychotherapy and counselling practice located in Sydney, Australia. By accessing or using our services, you agree to be bound by the following terms and conditions (the “Terms of Service” or “TOS”). If you do not agree to these TOS, you may not access or use our services.

Services
We provide psychotherapy and counselling services to individuals and groups, which may include individual therapy, couples therapy, family therapy, and group therapy. Our services are provided by qualified and licensed psychotherapists and counsellors who are registered with the Psychotherapy and Counselling Federation of Australian (PACFA) and adhere to the Code of Ethics of PACFA.

Service Agreement

This Service Agreement (“Agreement”) is made and entered into by and between Connecting 2 Minds Pty Ltd(“Practice”), and the client receiving services (“Client”).

Services: The Practice will provide the Client with psychotherapy and counselling services in accordance with the terms and conditions of this Agreement.

Appointments: Appointments are scheduled in advance and are typically 50-60 minutes in duration. We require at least 24 hours’ notice for cancellations or rescheduling of appointments. Appointments cancelled or rescheduled with less than 24 hours’ notice will be charged the full fee for the missed appointment. If you miss your appointment without notice, you will be charged the full fee for the missed appointment. In the case of emergency or illness, the full fee may be waived at the discretion of the practitioner. If you have a package or prepaid sessions, cancellations will result in the loss of that session. We reserve the right to charge a cancellation fee to clients who repeatedly miss or cancel appointments without adequate notice.

Scheduling: The Client will schedule appointments with the Practice by calling or emailing the Practice or booking online via Halaxy. The Practice will make every effort to accommodate the Client’s scheduling requests but cannot guarantee specific appointment times.

Fees: Our fees are based on the services provided and are in line with the PACFA recommended fees. Payment is due at the time of the appointment. The Practice accepts cash, credit card, eftpos, BPAY, and bank transfer as forms of payment. We do not bill private health funds or Medicare.

Confidentiality: The Practice will keep the Client’s information confidential and will not disclose any information about the Client or their treatment to any third party without the Client’s written consent, except as required by law.

Termination: Either party may terminate this Agreement at any time by providing written notice to the other party.

Governing Law: This Agreement will be governed by and construed in accordance with the laws of the State of New South Wales, without giving effect to any principles of conflicts of law.

Entire Agreement: This Agreement constitutes the entire agreement between the parties with respect to the subject matter hereof and supersedes all prior agreements and understandings, whether oral or written.

Modification: This Agreement may be modified only by a written instrument executed by both parties.
